Fundraising Form: Complete this for all fundraising activities and submit to O-Team for approval.
Request for Memorial Plaque: Complete and return to Church Administrator. The Administrator will let you see what is available on the Memorial Wall for placement.
Request for Payment: Used by Monitors and individuals requesting reimburse for items purchased for Church and/or payment for rental duties.
TUUC Auction Donation Form: Used by individuals donating items or services to the auction.
In-House Use of Space Form: Used by congregational members to secure space when needed for church events or meetings.
Rental Application Form: Complete and return to Church Administrator to rent space at TUUC.
